Application & Admissions Requirements

Foreign Applicants (INCLUDES U.S. CITIZENS):

All documents must be authenticated by the Philippine Embassy in the Country of origin. The following are mandatory

• NMAT Certificate required for continued matriculation but not for initial admission.
   
• Official copy of Transcript of Records from the College attended.
   
• Official copy of College Diploma or Certificate of Graduation
   
• Eight (8) photographs, size 2"x2", taken, at least, within two (2) months prior to application
   
• Clear Photocopy of Birth Certificate
   
• If married, clear Photocopy of Marriage Certificate
   
• Certificate of good moral character issued by two (2) former professors in the premedical course.
   
• Eight (8) copies of Personal History sheets from the Medicine Accounting Office or Philippine Embassy
• Two (2) copies of Affidavit of Financial Support by parents or guardians from country of origin
   
• Resident Aliens Only: two (2) Xerox copies of verified Asian Certificate & Registration
   
• Non-Resident Aliens Only: Study Permit and Student Visa, renewable every six (6) months. Documents obtained with assistance of the College upon Acceptance
• Two (2) copies of Letter of Recommendation
   
• Clear Photocopy of passport (Black & White)
   
• Eight (8) additional 2"x2" pictures
   
• Police Clearance from Country of Origin
   

Transfer Students/College Graduates:

• Certificate of Grades/Scholastic Records/Class Cards for initial evaluation
   
• Honorable Dismissal or Transfer Credentials
   
• Clear Photocopy of Birth Certificate
   
• Two (2) 2"x2" ID photos
   
• High School Graduates:
   
• Form 137A or High School report Card
   
• Certificate of Good Moral Character from the Principal or Counselor
   
• Clear Photocopy of Birth Certificate
   
• Clear Photocopy of High School Diploma
   
• Two (2) 2"x2" ID photos.
